About Welcome Baby: Road to Resilience
Welcome Baby: R2R is a free and voluntary program that supports families in Yolo County during pregnancy and after birth. Families are referred by a healthcare provider or community organization, or may also sign up themselves.
You will receive:
- An in-home nurse visit within one week of coming home with your newborn
- Up to two more visits from a Community Health Worker specializing in family resources (in-home or virtual)
Visits will include:
- Clinical assessments of mom and baby
- Breastfeeding support and referrals
- Connection to community resources
- Help scheduling appointments
- Health Education
- And more!

- Are pregnant or have a child under 2 years of age
- Live in Yolo County
